1. Patient Summary:
-Needs to be completed and signed by the dream recipients doctor.

2. GKTW Medical Authorization:
-Needs to be completed and signed by the dream recipient’s doctor.

3. GKTW Release:
-Fill in dream recipients name on page 1.
-Read pages 1-4.
-Publicity Release (Page 4): Complete information for each of the dream recipients parents/legal guardians.
-On the line below, state dream recipient and their siblings names.
-Publicity Release (Page 5): Fill out one of these forms for ALL other adults and minors attending the trip.
The parent/legal guardian information goes on the left and the minor (their child’s) information goes on the right.
4. GKTW Minor Release:
-Each family needs to complete one of these forms for each of their minors.
-Read and complete information at the bottom.
5. GKTW Adult Release:
-Each adult that is attending the trip needs to read and complete this form.
7. GKTW Absent Parent Form:
-Only needs to be filled out if the dream recipient has an absent parent.
